All club member activities fall under the rules and regulations of Cycling SA. We therefore propose the following:

1. The potential dangers of the sport are a reality. I’m aware of this.
2. I am aware that Cycling SA and its affiliated falls under the ruling of the UCI for events, the management and standards of ruling the sport. Cycling SA cannot ensure my safety. I undertake to:
3. Take part in the sport and all related activities in my own free will, taking into account the risks and responsibilities associated with the sport and the activities of cycling.
4. I undertake to hold no person or body involved in the organisation, sponsorship, or related activity under the ruling of Cycling SA responsible for any damages which I or any others, including dependents, may suffer as a result of any injury or death resulting from any act or omission on the part of such body or person, whether due to negligence (including gross negligence) or otherwise.
5. I undertake to comply with all safety rules and regulations of Cycling SA and its affiliated bodies, including the UCI (The Union Cycliste Internationale is the world governing body for cycling).
6. I undertake to wear all the safety equipment that conforms to the requirements of Cycling SA and its rules and regulations.
7. I had the opportunity to review the Anti-Doping rules associated with the sport. I therefore give:
8. Consent and agree to the rules associated with anti-doping, including but not limited to all amendments to the Anti-Doping rules and all International standards incorporated in the Anti-Doping campaign to keep the sport clean and cyclists healthy.

I also acknowledge and agree that any dispute arising out of a decision made pursuant to the UCI Anti-Doping rules, after exhaustion of the process provided to appeal to an appellate body for final binding and arbitration according to the Article associated with the Anti-Doping rules, will be in the Court of Arbitration for Sport of the International-Level for athletes.
10. I acknowledge and agree that Cycling SA has jurisdiction to impose sanctions as provided in the Anti-Doping rules.
11. I acknowledge and agree that the decisions of the arbitral appellate body referred to in this document shall be final and enforceable and that I will not bring any claim, arbitration, lawsuit, or litigation to another court or tribunal.
12. I hereby consent to the use of my image in official broadcast media or promotional use by Cycling SA and its affiliated clubs.
